The Vida Nueva is now online! Use the link below to submit an application for your candidate. You will still need to provide a signed copy of the application and medical wavier prior to the weekend but this will let us know quickly who’s coming.
You can use the link below to download the application. It is a fillable PDF the allows you to type in the information and save it on your computer. The completed form will need to be printed and signed. Please mail it to the address listed on the application.
